Option 1 — Delete from within the app
- Open the Dugan Psychiatry app and sign in.
- Tap Profile.
- Scroll to Delete Account.
- Confirm when prompted.
Option 2 — Request deletion by email
If you have removed the app or can no longer sign in, email info@duganpsychiatry.com from the email address on your account, with the subject line “Account deletion request”.Please include:
- your full name
- the email address or phone number registered to the account
What is deleted
When your account is deleted, we permanently remove:
- your account and sign-in credentials
- your profile information, including name, email address, phone number, and profile photo
- your messages with your care team, including any photos or files you shared
- your mood check-ins and assessment responses
- appointment records held in the app
- payment records held in the app
- push notification tokens and device identifiers
What is kept, and for how long
Your medical record is not deleted. Dugan Psychiatry is required by law to retain patient medical records, and those records are held in the practice’s medical record system, separately from the app. Deleting your app account removes your access to the app — it does not, and cannot, delete your clinical record.
Your medical record is retained for six years from your last date of service, as required by Arizona law.
If you wish to request access to or amendment of your medical record, contact the practice directly at info@duganpsychiatry.com. That process is separate from deleting your app account.
Payment information
Card details are never stored by Dugan Psychiatry. Payments are processed by Stripe, and card data is held by Stripe under its own terms.
Video visits
Video appointments are not recorded or stored. Audio and video are transmitted live between you and your provider for the duration of the visit only.
